Over 150 million people suffer from osteoporosis. Above all women. How does the disease come about? And how dangerous is it? Prof. Peter Pietschmann, osteoporosis expert at MedUni Vienna, will answer questions.

What exactly is osteoporosis?

“Osteoporosis is a very common bone disease,” says Pietschmann, as a result of which bone strength is reduced. This in turn increases the risk of bone fractures. “Of course every bone breaks sooner or later if very strong forces are applied,” explains the expert. If you suffer from osteoporosis, the risk that the bone will break is inherently higher.

How does osteoporosis arise?

Traditionally, osteoporosis is seen as a hormonal disease. The immune system is also likely to play an important role. “When there is acute inflammation in the body, bone loss usually also occurs. In other words, inflammation means bone loss.” Chronic inflammation damages the bones in the long term. In the whole body.

Are there different forms?

A basic distinction is made between the primary and secondary forms of osteoporosis. While the latter is triggered by other diseases or the use of certain medications – a typical example is the ingestion of a higher dose of glucocorticoids over a longer period of time – the cause of the primary form is not yet fully understood.

have behind them, and men. While in women it is primarily an overactivity of the bone-degrading cells that is responsible for osteoporosis, in men it is an underactivity of the bone-building cells that is more decisive. Overall, the primary form occurs more frequently than the secondary.

Who is most often affected?

Osteoporosis is two to three times more common in women than in men. Since it is a very common disease, it is not uncommon in men either. “In women, the onset of postmenopause is often an event that stimulates bone loss,” explains the expert. “With a 50-year-old, the probability that sooner or later an osteoporosis-related fracture will occur is at least 30 percent.”

How does osteoporosis show up?

“The mere fact that the bone changes negatively does not cause any complaints,” explains Pietschmann. That is a certain problem. It is therefore recommended, if certain circumstances exist, to undergo a bone density measurement from a certain age. This recommendation is aimed at women with an increased risk of osteoporosis over the age of 60. For all other women, a guideline age of 65 applies.

Is Osteoporosis Hereditary?

If your parents suffer from osteoporosis, the risk of developing the disease increases significantly. In fact, this aspect represents the greatest risk factor of all, as it is responsible for 50, if not up to 70 percent of osteoporosis cases. “If the parents develop osteoporosis, it is a warning sign,” warns the doctor.

How does the risk of osteoporosis increase?

Other risk factors – apart from age and gender – are smoking, excessive alcohol consumption, severely underweight and severely overweight and reduced mobility. An overactive thyroid, heart failure, liver disease or rheumatoid arthritis can promote the development of secondary osteoporosis.

Diabetes also increases the risk of illness. Interestingly, the bone density in type 2 diabetes patients is often normal, if not increased, despite the risk of osteoporosis. “This shows,” says the expert, “that bone density measurement, although it is a good instrument, is not perfect.” Ultimately, the fragility of the bones depends not only on the density, i.e. the quantity of the bone, but also on its quality.

Do drugs favor osteoporosis?

Taking certain medications can also increase the risk of developing osteoporosis. For example, drugs that belong to the class of glucocorticoids. The same applies to special drugs for epilepsy and various antidepressants.

How is the disease going?

“The disease develops over many years.” First and foremost, there is osteopenia, a reduction in bone density, the preliminary stage of osteoporosis, so to speak. Not everyone who suffers from osteopenia actually has to contract osteoporosis. As the disease progresses, fractures occur. “First the bone density and structure decrease, then the typical fractures occur,” summarizes the doctor.

Which bones break most often?

Pietschmann speaks of “three classics”: the femoral neck fracture, the radius fracture – here the forearm near the wrist is affected – and the vertebral body fracture. The latter is most common in osteoporosis patients. “This is when the vertebral bodies collapse,” the expert explains the painful incident. In addition, fractures of the upper arm, the pelvic ring and the ribs can increase. In extreme cases, the ribs can break with a strong coughing attack.

How can you prevent?

A healthy lifestyle minimizes the risk of developing osteoporosis. In this context, Pietschmann calls a balanced diet – rich in

Vitamin D

and calcium. The latter mainly in the form of milk and dairy products. Sufficient exercise is of course also important. And because excessive alcohol consumption and smoking increase the risk of the disease, both of these factors must be avoided.

How is osteoporosis treated?

Drug treatment is essential in the event of illness. “A balanced diet alone would no longer be sufficient here,” said the doctor. Nevertheless, one should consume the recommended daily 1,000 milligrams of calcium through diet. Because this is the natural route of absorption and calcium tablets are not always so well tolerated.

Vitamin D, on the other hand, should be taken in the form of supplements. And then there are special osteoporosis drugs, especially those that inhibit bone loss. There are also those that have a bone-building effect. However, this is a very costly and very time-consuming therapy. For this reason, it is only used in very severe cases of osteoporosis.

quarantine is prescribed for those affected. The trauma psychologist Brigitte Lueger-Schuster explains how this exceptional situation affects the psyche and how families manage to make everyday life pleasant in quarantine at home.

In order to curb the spread of the coronavirus, the health authorities in Austria are allowed to order a home quarantine of 14 days. In Vienna and Lower Austria alone, more than 800 people are currently isolated from the outside world in their own four walls.

It is not known how many cases there are in all of Austria. When asked by the Ministry of Social Affairs, it said: “There is no complete overview of the Austria-wide quarantine cases.” In any case, it can be assumed that as the coronavirus spreads further, domestic quarantine will also increase.

Why conflicts inevitably arise

And dealing with this unfamiliar situation will be difficult for many Austrians. Because ordered house arrest harbors great potential for conflict. Apart from the practical requirements, meaning the increased hygiene measures and the daily care, confined spaces are demonstrably aggressive.

“So-called ‘density stress’ arises in tight spatial conditions. This discharges emotions and overreactions can occur,” explains the clinical health psychologist Brigitte Lueger-Schuster to news.at. If the isolation situation lasts too long, helplessness spreads among those affected, which can even lead to acts of violence.

Trapped at home – China showed us how

This is also confirmed by a look at China: everyday life in quarantine led to increasing cases of domestic violence there. “As a result of the epidemic, many couples sit on each other at home for more than a month, creating a lot of subliminal conflict,” a local official said in the Global Times.

Discipline is required

Of course, Austria is still a long way from this dubious side effect of everyday life in quarantine at home. After all, the officially ordered house arrest lasts “only” 14 days if there is suspicion of infection in this country. The professor from the Faculty of Psychology at the University of Vienna appeases: “Two weeks is not really long and it goes by quickly. The families will definitely be able to cope with it.”

But she doesn’t want to downplay the situation. Because in quarantine at home, something can of course come to light that usually happens on long-awaited vacation. “When you finally have time for each other, you realize that many things in a partnership or in family life are not going well,” says psychologist Lueger-Schuster. And this realization is a burden.

Practical tips for living in quarantine

So how can you prevent quarantine quarreling from breaking out? And how do you make everyday life pleasant?

The most important thing is to plan the day well. It can be useful to write down the activities or at a family conference to give everyone the chance to come up with suggestions.

“The more active you are, the shorter the hours,” says the expert. Whether playing cards, cooking, watching series, writing a diary, tidying up or doing gymnastics, there are many ways to get through the day well.

Whine less, do more!

Things that you have wanted to do for a long time and never had the time to do so could be done in quarantine. “We are often distracted in our hectic everyday life. Now you have two weeks to really tackle these plans.” The less space there was to feel sorry for yourself and to complain, the faster the hours would go by.
